What actually changes the price
Five things move the number: diameter, thickness, metal finish, how much enamel colour fill the design uses, and quantity. A 50mm medal with two enamel colours sits at a very different price to a 70mm shaped medal with full colour fill and a spinner element.
Ribbon choice matters too. A plain colour ribbon costs less than a fully printed bespoke one, though printed ribbons are usually the cheapest way to make a medal feel expensive.
